Policy & Procedures



At this time, Collaborative Family Solutions has elected to contract with Medicaid/HuskyAnthem/Blue Cross Blue Shield, and Optum/UnitedHealth Group/ConnectiCare  insurances.  If you have a different insurance company that you would like to use, an out-of-network claim may be completed. 

Out-of-Network Benefits: You may have out-of-network benefits and an out-of-network claim may be submitted on your behalf at your request. Please call your insurance provider directly to determine what your plan offers.

Telehealth Services:  At this time,  Anthem/Blue Cross Blue Shield insurance has permitted use of telehealth therapy sessions utilizing doxy.me teleconferencing platform. Please call your insurance to determine if your plan covers telehealth services.

An in-person intake is required before determining clinical appropriateness of telehealth services.


Cancellation Policy: To avoid a $25 fee, notify us at least 24 hours prior to your appointment to cancel or reschedule. Reoccurring cancellations and/or no-shows may result in termination of treatment.

Payment Policy: Payment is due at the beginning of each appointment. CFS accepts all major credit cards, check, and cash.

If for any reason insurance denies coverage, you are responsible for the full fee. This does not apply to Medicaid/Husky due to current Medicaid/Husky policies.

Receipts and superbills are available upon request.

Against Clinical Advice Policy: A clinical recommendation for treatment will be provided following the intake. As previously mentioned, sometimes outpatient is not clinically appropriate and a higher level of care is required. Please note that you will be required to sign an Against Clinical Advice (ACA) letter if the recommendation is not followed.
